Grade 4 and 5 Summer Math Work As you know it is very important to keep up on math skills over the summer.  Therefore it will be required that each student do a minimum of 12 sessions of 30 minutes each on one of the following web-sites.  Use the log form linked to below to keep track.  The student and parent must sign the log, this sheet will be collected the first day of school, and be graded towards your first trimester grade. (The timing averages 30 min. each week of summer).

Summer Reading:  Grade 4

Summer Reading:  Grade 5

Two Book Reports are due on the first day of school.  You will have one assigned book and one book of choice.  The book of choice should be on grade level and at least 80 pages in length.  The other book will be Shiloh by Phyllis Reynolds Naylor.   Download the complete form for full instructions.  Grade 5 summer reading form.

Supply List:  Grade 4 & 5

• Accordion folder (with at least 8 slots) please label with child’s name   

• Notebooks:   one subject all must have spiral binding, perforated pages and be college Ruled.  Please label with subject and child’s name

  • Reading--yellow                        

  • Science--green                        

  • Math--red                        

  • Social Studies--blue                        

  • Spanish--black

• Must have at least two of the following EVERY DAY:  

  • #2 pencils with erasers  

  • erasable pens (blue or black ink)  

  • red correcting pen  

• glue stick  

• 1 yellow highlighter  

• 1 blue highlighter  

• pair of scissors  

• ruler  

• 6 of the 2” metal binder clips  

• 1 black sharpie marker  

• 1 black expo dry erase marker and eraser  

• personal dictionary/thesaurus  please label with name on inside cover  

• box of colored pencils  

• spiral bound 3x5 notecards  

** for the classroom—2 boxes of tissues and one container of anti-bacterial wipes
